Life has been easier since the advent of the internet but it is also a bane. The beauty industry is unquestionably one of the hardest hits. The mushrooming number of social media influencers and sponsored content published online has transformed people’s perceptions of beauty, resulting mostly in unrealistic beauty standards. South Korea and the United States have consistently ranked first in plastic surgery and facial remodelling. How far is India in its efforts to firmly establish a beauty standard, or will it ever change?

With a comprehensive background in ophthalmology and extensive training in both India and the United States, let’s find out what Dr. Sneha Shah, an aesthetic surgeon, has to say about the evolution of beauty trends in India.

Why is facial remodelling necessary?

Dr. Shah: Facial remodelling is commonly undertaken for managing signs of ageing, and addressing scars and defects from trauma or birth. It can be a part of the gender confirmation process. It may also be done to enhance self-esteem and for social and professional reasons. Although it is a personal choice, it must be embarked upon, after consultation with a professional and understanding all the facets of the treatment.

In recent years, looking ideal or following a specific trend has become quite important for a certain age group. In comparison to other Asian countries where following a beauty standard is under debate, how far is India from significant facial reconstruction?

Dr. Shah: India has an ethnically diverse population, hence having a beauty standard is not possible. Historically, we have looked at the West, as a benchmark. The Millenials and Gen Z  are being more and more influenced by the East via exposure to media. It’s not just women, but also men who are seeking these treatments and this is only going to increase.

But still, I would say India still has a long way to go. While minimally invasive procedures like fillers, botox, dermabrasion, and vampire facials have become commonplace, the acceptability of surgery is only now starting to increase.  

What is the most common reconstruction procedure that people demand?

Dr. Shah: As an Oculoplastic surgeon, most commonly I get queries for Blepharoplasty, which is an under-eye bags procedure, Ptosis correction which is a procedure for droopy eyelids, and xanthelasma treatment.

Tell us the intersection of artistry and medical expertise in facial aesthetics, as seen through your practice.

Dr. Shah: It’s funny you should ask this question, because the opening slide for many of my presentations reads, “Where science meets art”. If we look at the history of facial aesthetics and its journey over time, it has evolved greatly. Earlier we thought that art follows no rules, but with greater understanding, we realise that everything, even nature follows rules. Like for example, the golden ratio, which is present everywhere in nature, and makes everything look beautiful. Understanding this and in turn, applying it scientifically to facial aesthetics has made us better at achieving our goals.

What are the latest advancements in oculoplastic surgery and facial aesthetics and how it enhance patient outcomes?

Dr. Shah: There are plenty of new technologies, simple and complex. Some apps can predict the way you will age, and what you may look like when you are say 40 or 70. Similarly, apps can mimic what one can expect after the use of a filler or botox in a particular area. These help greatly in patient counselling, especially initiating treatment-naive patients, as they can now visualise the expected result.

Then there are the more advanced technologies such as 3D imaging and 3D printing, GPS, and navigation systems making procedures more precise, safe, and accurate.

We are aware of innumerable instances where the facial reconstruction process has gone wrong. How would you advise someone looking for the best surgeon and treatment plan?

Dr. Shah: Well, Google is both a boon and a bane. It increases awareness of the procedure and its potential for complications. When we are counselling patients and taking their consent for the procedure, we always explain to them the possibility of complications and their probability. I encourage them to read all the provided information and ask as many questions before the surgery. Choose surgeons based on their qualifications, training, and expertise. Pre and post-pictures are a good guide. The course of action is finally decided by the patient and the surgeon after consultation and discussion.

What is your outlook for the practice of facial reconstruction in India over the next few years?

Dr. Shah: It is definitely on a rising curve, there are no two questions about that. There are two aspects. Firstly, with growing awareness, accessibility, and affordability, Indians can go beyond what is essential, and pay more attention to health and beauty. Secondly, India is growing as a hub for facial aesthetics and reconstruction, receiving patients from across the globe.

What are your ideas on current beauty standards and whether they are vital to follow?

Dr. Shah: Beauty standards are constantly evolving as is our understanding of beauty. Though the “latest trend” may dictate the demand and provide the flavour, the essence of treatments should focus on creating a healthy, natural look. In my practice, the mantra is “A Youthful You” and I think one should not lose track of that.

I have personally been practicing facial yoga for years now and it works like magic, the results though not instant it very effective and long-lasting. Yoga for firm face and glowing skin

* Start with reciting the vowels – A, E, I, O, U. Say the vowels in an exaggerated manner so that you stretch all your face & neck muscles. To begin with, say the 5 vowels each 10 times in a day, then slowly build it to 20-30 times in a day. This is a complete exercise for your face.

* The second exercise that you can do is, close your fist and place it under your chin. Now try opening your mouth while pressing your fist up to close the mouth. This is a great exercise to get rid of the double chin. Once you’ll start doing it, you will feel the stress on your jawline and who doesn’t want the perfect jawline?

* A perfect exercise to avoid getting wrinkles under your eyes and on your forehead is to place both your hands on your forehead so that no muscles move. Now blink your eyes in an exaggerated manner about 10 times. Blinking will strengthen your eye muscles.

* Another exercise that you can try for strengthening your eye muscles is by placing your index finger and your middle finger on either side of your eyes, one at the inner corner and the other at the outer corner, and blink. Do this 10 times a day.

* Chubby cheeks look super cute when we are kids but become a major concern when we grow up. To pull up saggy and chubby cheeks, pull everything to your side. It’s like doing a sideway kiss, this stretches your cheek muscles and helps in making them firm.

* We all worry about our laughing lines and as we grow up, they tend to become more visible. The perfect solution to reducing laughing lines is placing your fingers firmly over your laugh lines, now smile as widely as possible, and remember to keep your lips apart. Hold this for 5-10 seconds and repeat the exercise 20-25 times. This will help to remove creases around the cheek and lift your cheeks.

* A major thing to remember while applying your daily moisturizer is to always apply in an upward motion, especially around the cheeks and on the neck. If you do it otherwise you are pulling your skin down making it saggy. Another exercise that you can try is, placing your thumbs under your chin and massaging your cheeks in an upward motion (ending at the temples) using your fingers. Do this 5 times. This helps smoothen smile lines and promotes blood circulation.

Exfoliates dead skin – Lemon juice contains alpha-hydroxy acids (AHA) such as glycolic acid. Such acids are commonly used in skin care, as they increase cell turnover and slough away dead skin cells. As a result, many folks use lemon juice in hopes of reducing dullness and brightening the skin.

Reduces Acne – Lemon juice has astringent qualities due to its acidic level. Ingredients with a low pH level can help decrease inflammation and oil that may contribute to the formation of acne. Furthermore, citric acid, a type of alpha hydroxy acid (AHA), can help break down dead skin cells that lead to noninflammatory forms of acne, like blackheads.

Increases collagen production – The body needs vitamin C to synthesize collagen, the main structural protein in the skin. Using vitamin-C-rich ingredients can help support your intake of the nutrient, and ultimately, collagen production.

Skin spot or hair lightening – Citrus ingredients like lemon may also work well on lightening age spots or acne scars, as well as any hair on your face.

Dandruff treatment – lemon has been used to treat dandruff for ages, the sloughing-off effects are attributed to lemon’s natural levels of citric acid, as AHAs have an exfoliating effect on the skin, therefore it can also alleviate skin patches attributed to dandruff.

Offers antioxidants – Vitamin C is a potent antioxidant, meaning it can help fight oxidative stress. This is noteworthy because oxidative stress can lead to inflammation, sagging, and faster skin aging. However, lemon juice may help stave off these effects because it contains vitamin C.

Now let us look at some disadvantages of using lemon on your face:

Cause irritation: Even if your stint with lemon juice doesn’t result in burns, the ingredient can still cause irritation. This is a side effect of acids, like those in lemon juice, weakening and damaging the skin barrier. According to Worden, possible symptoms include peeling, dryness, stinging, and redness, depending on your skin tone.

Sunburn: Citrus fruits applied topically can also increase your risk of sunburn. Never apply lemon before going outside in direct sunlight, and don’t use it for several days before any planned outdoor activities.

Phytophotodermatitis: It is a type of skin reaction to citrus fruits and other culprits, like parsley, celery, and carrot plants. When you have citrus substances on your skin and your skin is then exposed to UV rays, an inflammatory reaction may occur. This can result in swelling, bumps on your skin, redness, and rashes.

Hyperpigmentation: Although lemon juice is often used to reduce hyperpigmentation, it can actually worsen the issue. That’s because the sunburns caused by lemon juice can cause blistering, leading to months of hyperpigmentation and potentially permanent scarring. Basically, you need to understand that vitamin C in beauty products is different from vitamin C in your food.

We are not saying don’t use lemon at all on your face, but do the following so that you don’t develop any skin problems:

Always do a patch test before applying it to your face. You can do it on your arms and wait for some time to see the reaction

Always dilute lemon juice with something like rose water or honey, avoid using it alone and directly on your face

Sunscreen is a must after you apply lemon juice

Use fresh lemons in case you are adding the juice to a pack
