How to take care of children with eczema

The best emollients for Eczema trial has found that no one type of moisturizer is better than another for children, according to a new study by the University of Bristol.

The findings of the research were published in the journal ‘The Lancet Child & Adolescent Health’.

This study, the first in the world to directly compare different types of moisturizers, highlights the importance of patient education and choice when deciding which moisturizers to use for children with eczema.

Moisturizers (also called emollients) are recommended for the one in five children who have eczema (also known as atopic eczema/dermatitis), which causes dry and itchy skin. Over 100 different moisturizers are prescribed in the NHS, costing over £100 million a year. Lack of research in this area means NHS guidelines vary widely in what is recommended, which leads to confusion and waste.

In the study, led by the universities of Bristol, Nottingham and Southampton, 550 children with eczema aged under 12 years were randomized to use one of four types of moisturizer (lotion, cream, gel or ointment) as their main moisturizer for 16 weeks. Parents completed diaries about their child’s eczema for a year, and some were interviewed to gain an in-depth understanding of how they used the moisturizers and what they thought of them. All children also had an independent examination of their skin. Used alongside other eczema treatments, there was no difference in the effectiveness of the four types of moisturizer used in the study. Skin reactions such as itching or redness were common with all moisturizer types. Awareness of the different types of moisturizers was low, and users had different preferences based on how the moisturizers look and feel. For example, some people liked how lotions quickly soaked in whereas others preferred the “barrier” provided by ointments.

Professor Matthew Ridd, a GP and study lead from Centre for Academic Primary Care at the University of Bristol, said: “A study of this type has been long overdue. It has not been in the interest of the manufacturers to directly compare types of moisturizer in the way we have done in this trial. Our findings challenge conventions about how often moisturizers need to be applied, which types are less likely to cause problems and which patients should be recommended certain types. For example, ointments are often suggested for more severe eczema, yet they were found to be no better.”

Hayley, the mother of Abriarna who took part in the trial, said: “The trial helped us find an emollient we had never used before, which has helped keep Abriarna’s skin in good condition for the longest time. Her eczema often requires a different emollient for various purposes, for example, Abriarna has a regular day-to-day moisturizer, which we found through the trial. She then has an emollient for days when her eczema is agitated and then an emollient to wash in as well as to use after handwashing.”

What this study does is emphasize the importance of having the four main types of moisturizers available on formularies for children with eczema, so that the right product can be used at the right time.”

Professor Hywel Williams, consultant dermatologist and co-researcher at the University of Nottingham, explained: “Along with anti-inflammatory treatments such as topical corticosteroids, emollients are a really key part of treatment for childhood eczema, preventing flares and helping to soothe the skin and improving the quality of life for children and their carers.

“Our study shows that one size does not fit all, and points to the need for doctors to make parents aware of the different emollient types and to help them choose which one is mostly likely to work for them. At last we have evidence that supports the saying, ‘The best moisturizers are the ones the patient will use.'”

Professor Nick Levell, NIHR National Specialty Lead for Dermatology, said: “Around one in five children get eczema and the scratching and sleep loss affects school performance and exhausts children and their parents. This study confirms that parent and patient preference is very important in choosing a moisturizer to treat eczema. Some people prefer ointments, but others like gels, creams or lotions. No one option is best. As reactions to moisturizers are common, it is important that the NHS provides a wide choice to help parents find something that soothes and calms their child’s fiery skin.” (ANI)

STUDY: Toxic elements in imported creams

According to the World Health Organisation (WHO), mercury is hazardous to health and has the potential to cause damage to the central nervous system, the gastrointestinal system and the kidneys…reports Asian Lite News.

A study of 15 known brands selling imported skin whitening creams in the Indian markets has found that they contained dangerously high levels of mercury, a heavy metal and neurotoxin.

“A total of 15 skin whitening creams of different brands manufactured in India, Pakistan and Philippines were enlisted for the study. Samples were collected from different cities, i.e., Mumbai, Vijayawada, Delhi and Thiruvananthapuram and tested at the Shriram Institute for Industrial Research in Delhi using Inductively-Coupled Plasma-Mass Spectrometry (ICP- MS) by a standardized method,” Toxics Link said in its latest report “Dark truth of skin whitening creams: Presence of Mercury in skin whitening creams”.

Toxics Link is a Delhi-based environmental research organisation working on the issue of waste and chemicals for the last 20 years. Toxics Link is part of the global alliance of zero mercury Working Group and involved in this mercury policy project.

The Zero Mercury Working Group (ZMWG) is an international coalition of more than 110 public interest environmental and health NGOs from 55 countries from around the world formed in 2005 by the European Environmental Bureau and the Mercury Policy Project.

According to the World Health Organisation (WHO), mercury is hazardous to health and has the potential to cause damage to the central nervous system, the gastrointestinal system and the kidneys.

The study detected mercury in six out of 15 samples; five samples had alarming levels of mercury ranging from 4,000 to 14,000 ppm (parts per million) and only one out of six samples had mercury concentration below 1 ppm i.e., 0.3 ppm, a release from the Toxics Link said.

The study also revealed that all the six samples containing mercury were manufactured in Pakistan, according to the labels on the packaging. “However, the test results revealed that all the samples manufactured in India had mercury below detectable limits,” it said.

India has a stringent regulation, under the Drugs and Cosmetics Rules 2020, to prevent mercury from being added to cosmetics. Manufacturing and import of cosmetics containing mercury compounds are completely prohibited; but unintentional mercury is allowed up to 1 ppm.

Chandni Whitening Cream, Fresh Face, Faiza Beauty Cream, Golden Pearl Whitening Beauty Cream, Goree Day and Night Whitening Cream, Goree Beauty Cream (all from Pakistan) Kojie San (Philippines), Lotus Herbal White Glow Skin Whitening and Brightening Gel, Face Crema (India) are some of the brands.

However, the present study is an indicator that the mercury-containing products are still available in the Indian market. India has also ratified the Minamata Convention on Mercury, which restricts mercury use in cosmetics to 1 ppm.

“It is disturbing that the mercury concentrations in some of the imported samples were found to be as high as 1,000 times more than the current permissible limit of 1 ppm. It is also important to mention that a previous study by Toxics link in 2019 had also reported high mercury concentrations in the same brands. For that study, released on Thursday, the samples were purchased from the online portals Amazon and Flipkart,” the release said.

“Availability of highly toxic and non-compliant products in shops and from online portals are serious breach of the national rules and need to be acted upon immediately by the regulators,” said Associate Director, Toxics Link, Satish Sinha.

The use of these products could result in severe health risks. Therefore, there is an urgent need for periodic monitoring of imported skin whitening creams by the regulatory authorities, to safeguard the health of the consumers. “A proper surveillance system needs to be in place to prevent high mercury-containing toxic skin whitening creams from being imported into India,” Senior Program coordinator, Toxics Link, Piyush Mohapatra said.

I only need to use sunscreen in summers

Ensure that you use a lightweight or gel-based moisturiser for oily skin, and you are good to go.

A pea-sized amount of product is enough for my skin

Unless a product asks to use a small dollop, you must use as much as required to reap the benefits. It is not a one-size-fits-all situation with facial care beauty products. Some spread easily, while others need to be used in thick layers to work more efficiently.
